1. Acceptance of Terms

These Terms & Conditions (the “Terms”) govern your use of the Southern Imaging Services website at sisimaging.com (the “Site”). By accessing the Site, submitting an online form, or otherwise interacting with content made available here, you agree to be bound by these Terms and by our Privacy Policy. If you do not agree, please discontinue use of the Site.

2. About Our Services

Southern Imaging Services (“SIS,” “we,” “our,” or “us”) operates outpatient diagnostic imaging facilities in North Carolina and South Carolina, providing MRI, CT, X-ray and related diagnostic imaging services. The Site is informational only and is intended to help patients, attorneys, and referring providers learn about our locations, services, and scheduling options. Use of the Site does not establish a physician–patient relationship and is not a substitute for evaluation, diagnosis, or treatment by a qualified, licensed health care provider.

3. Not Medical Advice

Content on the Site — including blog articles, service pages, FAQs, and educational materials — is provided for general informational purposes only and is not medical advice. It is not intended to replace consultation with, or examination by, a licensed health care professional. 4. Appointment Scheduling

Online scheduling forms, contact forms, and request-records forms submitted through the Site are requests for service, not confirmed appointments. A member of our staff will follow up by phone or email to verify availability, insurance or attorney-funding details, imaging orders, and any preparation instructions before an appointment is finalized. Information you submit through these forms is handled in accordance with our Privacy Policy and applicable state and federal law, including HIPAA.

5. Intellectual Property

All content on the Site — including the “Southern Imaging Services” name, logos, marks, page copy, photographs, illustrations, video, and code — is owned by SIS or its licensors and is protected by United States and international intellectual-property laws. You may view and print content for your own personal, non-commercial reference. You may not copy, republish, redistribute, modify, frame, scrape, or otherwise exploit any portion of the Site for commercial purposes without our prior written consent.

6. User Conduct

When using the Site, you agree not to:

• use the Site for any unlawful, fraudulent, or harmful purpose;
• submit false, misleading, or impersonating information through any form;
• attempt to gain unauthorized access to any portion of the Site, our servers, or any connected systems;

To the fullest extent permitted by applicable law, SIS, its affiliates, and their respective officers, employees, and agents will not be liable for any indirect, incidental, special, consequential, exemplary, or punitive damages — including, without limitation, damages for lost profits, lost data, business interruption, or personal injury — arising out of or relating to your access to or use of the Site, even if SIS has been advised of the possibility of such damages. In no event will SIS’s aggregate liability arising out of these Terms or your use of the Site exceed one hundred U.S. dollars (USD $100). Your continued use of the Site after a revision becomes effective constitutes your acceptance of the revised Terms. If you do not agree to the revised Terms, you must stop using the Site.

11. Governing Law

These Terms and any dispute arising out of or relating to your use of the Site will be governed by and construed in accordance with the laws of the State of North Carolina, without regard to its conflict-of-laws principles. You agree that any action at law or in equity arising out of these Terms or the Site will be filed only in the state or federal courts located in North Carolina, and you consent to the personal jurisdiction of those courts.
